As a service to our patients, we will file your insurance forms and assist you in understanding your insurance benefits.
We will discuss with you the cost of the different treatment options, any priorities, and help you make the best choice for you, while maximizing your dental coverage.

Most insurance plans are only intended to assist with the cost of treatment and seldom offer full coverage. 
We will estimate and provide your expected portion to the best of our ability.  

Please be prepared to pay your estimated portion at the time services are rendered.

Appointment times are very important.  This time is reserved for you.  There may be a charge for any appointment canceled or broken without a 48 hour advanced notice.

A 1.5% late charge (18% APR) may be added to an account if payments are overdue by 60 days.
